If you live in the practice area and would like to become a patient of the practice, please open the tab to download the Application Form or alternatively you can obtain one from Front Reception at either site.
Once you have completed the Application Form please forward it to us together with your signed Medical Card. If you have lost your Medical Card please ask us for a HS200 form. If you have never had a UK Medical Card click on the 'BSO Forms' tab to download a HS22X form.
Please note that you may see any doctor within the practice not just the doctor you are officially registered with.
In order to obtain a medical card you need to fill in an application form, which is called HS22X. This form is available in English and in a range of other languages. Please click on the respective links below to download the form in English and/or in translation.

Once you are registered with the Practice the following questionnaire should be completed and returned to us as soon as possible. This is particularly important if you are currently taking repeat medication.
In addition you may wish to arrange to have a New Patient Medical with one of our Healthcare Assistants.
